Enter your business telephone number and the address where records are maintained, if different from the preprinted address.
List the names, titles, social security numbers, and addresses of all principal corporate officers or partners.
For leased vehicles, include the manufacturer’s serial number and owner’s details as required.
Ensure that the application is signed by an authorized individual responsible for its accuracy.
Mark an X in the 'Do not renew' box for any vehicles that do not require renewal permits and adjust weights as necessary.
