  4. Proceed to categorize the patient’s treatment level by selecting one of the three options: Full Support, DNR Comfort Care (DNRCC), or DNR Comfort Care - Arrest (DNRCC - Arrest).
  5. For patients with a portable DNR order, ensure you attach a photocopy of this order within the medical record section of the form.
  6. Finally, have a physician sign off on the completed form to validate it. This step is crucial for compliance and must be documented properly.

Do-Not-Resuscitate: Comfort Care Arrest (DNR-CCA) Healthare personnel will provide all life-saving treatments unless your heart or breathing stops. Once your heart or breathing stops, only comfort care, or treatments that will ease pain and suffering, will be provided. CPR will not be initiated.
DNR. C1. Goal: Reverse medical problems or sustain life, with transfer to acute care AND assessment for critical care interventions WITHOUT intubation. Non-invasive ventilation may be offered and is noted in Section 3.
A DNR only goes into effect when the patients heart stops or they stop breathing. A Living Will, in contrast, is a legal document that covers a wide range of specifications around end of life care and planning.
DNR means that no CPR (chest compressions, cardiac drugs, or placement of a breathing tube) will be performed. A DNI or Do Not Intubate order means that chest compressions and cardiac drugs may be used, but no breathing tube will be placed.
