Diploma Order Form - uhd 2026

1. Click ‘Get Form’ to open the Diploma Order Form in our editor.
2. Enter your UHD ID Number in the designated field to identify your records.
3. Pay the diploma/graduation fee of $50.00 per diploma at the Cashier’s Office, either in person, via fax, or by mail.
4. Print your name exactly as it should appear on your diploma in the specified format (e.g., FIRST MIDDLE LAST). Ensure this matches your academic records.
5. Provide a mailing address where you would like your diploma sent, including number/street/apt, city, state, and zip code.
6. Indicate how many diplomas you wish to order and specify your anticipated graduation date.
7. Fill in your degree(s) and major(s) as they should appear on the diploma. Only include majors indicated on the diploma.
8. Complete your contact information including email address and phone numbers.
9. Sign and date the form before submitting it along with your payment receipt to the Registrar’s Office N-330.
