2. In Section A, begin by entering your name, employee number, base, job title, phone number, and email address. Ensure all information is accurate for proper processing.
3. Fill in the absence begin date and the name of your health care provider. This information is crucial for verifying your illness.
4. Sign and date the form at the bottom of Section A. Remember that alterations to the health care provider's information are not permitted.
5. If applicable, review Section B which must be completed by your treating health care provider. They will provide details about your condition and return-to-work status.
