Begin by filling out Section 1, which includes the Title of the Training Programme, Duration in Weeks, and ERD Code. Ensure all fields are completed accurately.
In Section 2, provide details about the Ministry and Agency associated with the nominee.
Section 3 requires personal information about the nominee. Enter the Name (with family name first), Sex, National Identity Card Number, Passport Number, Present Designation, and Management Grade. Be sure to check the appropriate box for designation level.
Complete Sections 4 through 11 by entering contact information, date of birth, years of service, educational qualifications, and training received. Use abbreviations where necessary.
Finally, in Section 12, ensure that you sign and date the declaration confirming that all provided information is accurate.
