Click ‘Get Form’ to open the disposition form in the editor.
Begin by filling out the Establishment Information section. Enter the Establishment Name, Address, ID Number, Responsible Person, Date, and Title accurately.
Proceed to the Description of Product(s) section. Specify the Type of Product(s), Facility Type (if applicable), Quantity, and Status (choose from Fresh, Frozen, Cooked, or Raw). Indicate if photographs were taken.
In the Conditions Requiring Action section, check any applicable boxes for suspected conditions such as Adulterated or Misbranded.
For Disposition of Product(s), select the appropriate action taken. If opting for voluntary disposal, confirm your agreement and specify how you will dispose of the products. Ensure to sign and provide your title.
If applicable, complete the Notice of Embargo section by checking relevant conditions and providing necessary signatures and dates.
