3. Provide your contact information including PESEL number, phone number, mobile number, email address, preferred airport, permanent address, and temporary address. Lastly, fill in the next of kin's details including their name, address, relationship to you, and phone number.
4. Move on to the 'Education' section. List your school name and address along with the dates attended (from and to).
5. In the 'Licences' section, specify the type of documents you hold such as Certificate of Competency or Passport. Fill in issue date, expiry date, document number, and place of issue for each.
6. Complete the 'Courses' section by entering course names along with their issue dates and expiry dates.
7. Indicate your language proficiency in the 'Languages' section by selecting from Excellent to Poor for each language listed.
8. Finally, provide details about your sea service including company name, vessel name and rank along with sign-on/sign-off dates and vessel specifications.
9. Review all entries for accuracy before signing at the bottom where you authorize processing of your personal data.
