The ID Badging Office is located at the basement level of Elevator Center 6 (select the Terminals 1, 2 3 button). At the basement level there are directional signs to the ID Badging Office.
Your badge type requires an FBI fingerprint background check and TSA security threat assessment. These must be completed before the badging process can continue. A warrants check will also be conducted. If you have any outstanding warrants, they must be resolved before the badge can be issued.

Replacement of an ID badge requires a completed application marked Lost or Stolen, the confirmation number obtained by the Communications Center, and two forms of ID. The mandatory replacement fee for a lost or stolen badge is $50.00; the fee for a subsequent lost or stolen badge within a 24-month period is $100.00.
