Fill out a Request for Immunization Records form and include your contact information in case we have any questions about your request. Please note that we keep immunization records on file 7 years from the date of admission.
How to submit your form: Email copies to healthservices@bmcc.cuny.edu. OR. Drop off in person at the Health Services Office in Room N-380, 199 Chambers Street. OR. Fax copies to +1 (212) 220-2367.
You will need to download and complete an MMR/Meningitis immunization form before you can register for classes. You will need to submit proof of two doses of measles, one of mumps and one of rubella. Submit a PDF via email to healthservices@bmcc.cuny.edu.
The Psychological Center at The City College of New York (CCNY), also known as the Clinic, opened in 1969. The Clinic provides low-to-moderate-cost services to the CCNY and West Harlem communities. The Clinic is also the primary practicum site for doctoral students in the Ph. D.
