5. In Section B: Departmental Acknowledgment, ensure that a copy of your military orders is attached. Your supervisor will need to sign and date this section after confirming eligibility for differential pay.
6. Section C: Benefits Department Acknowledgment requires confirmation of any life insurance or benefits coverage. The Leave Coordinator will also need to sign this section.
7. Once completed, submit the form to the Leave Administration Unit using the provided mailing address or fax number.
