1. Click ‘Get Form’ to open the Dog License Application in the editor.
2. Begin by filling out the Owner Information section. Enter your name, address, mailing address (if different), telephone number, and email address.
3. Next, move to the Dog Information section. Provide your dog's name, breed, color, rabies expiration date, and rabies tag number. Ensure that the rabies vaccine is valid at the time of licensing.
4. Select the appropriate box that describes your dog’s status: Puppy (4 to 7 months), Male (Unaltered/Neutered), Female (Unaltered/Spayed), or if you are a Senior Citizen Owner (age 65+).
5. Indicate if you are applying for a Group License (5 or more dogs) and review the fee structure provided for each option.
6. Once completed, save your form and choose your submission method: in person, mail-in, or drop box. If mailing or using a drop box, ensure to make out a check to 'Town of Hopkinton'.
