4. If you are changing your default bank account, repeat the new account details in both Part A and Part B. Clearly indicate which account should be set as the default for future transactions.
5. For deletion requests in Section C, specify the Bank Account Numbers and Names of accounts you wish to delete. Remember to attach a cancelled cheque or certified bank statement for verification.
6. Finally, review all entries for accuracy and ensure all necessary signatures are provided before submitting your form.
