1. Click ‘Get Form’ to open the college credential request in the editor.
2. Begin by filling out your personal identification details. Enter your first name, last name, student ID, date of birth, phone number, and email address in the designated fields.
3. Select the program you completed from the options provided: 1-year certificate, 1-year graduate certificate, 2-year diploma, or 3-year diploma. Specify the years you attended by entering the start and end dates.
4. If you wish to have your credential mailed to you, complete the optional mailing instructions section with your full mailing address. Choose whether you want a pick-up or mailing option for your credential.
5. Sign and date the form at the bottom to confirm your request. Ensure all information is accurate before submission.
6. Submit your completed form via email to either oshawaenrolmentservices@durhamcollege.ca or whitbyenrolmentservices@durhamcollege.ca as instructed.
