4. Proceed to Section B where you will select one action regarding your major or minor changes. Fill in the new major and option as applicable, ensuring that you obtain necessary signatures from your department advisor and chair.
5. Complete Part II - Department Review by providing your cumulative GPA and current units earned. Your advisor will need to sign here after advising you on requirements.
6. If applicable, obtain the Dean of Undergraduate Studies' signature in Section D if your new academic plan exceeds 144 units.
