1. Click ‘Get Form’ to open it in the editor.
2. Begin with Part I: Student Information. Fill in your ID number, full name, present classification (Sophomore, Junior, Senior), expected graduation date, current advisor, and the semester/year you started at Trinity.
3. Move to Part II: Major. Indicate your requested major, degree program (BA, BS, BM), and any applicable concentration.
4. In Part III: Interdisciplinary Cluster, select your chosen cluster from the provided list. If opting for a Student-Designed Interdisciplinary Cluster (SDIC), check the appropriate box and ensure you complete the necessary form online.
5. Sign and date the form in the designated area before submitting it along with your transcript to the chair of your academic department.
6. Once approved by the department chair, return the Declaration of Major form to the Registrar’s Office.
7. Lastly, complete the First-year Advising Program Survey online by the specified deadlines for Fall or Spring submissions.
