2. Select the semester your child will be attending by checking either Semester 1 or Semester 2.
3. Fill in the donation amount, which is $20.00 per semester, and provide the date for receipt.
4. Complete the Youth Information section by entering your child's Student ID, Last Name, First Name, Date of Birth, Age, and Grade Entering.
5. Indicate if your child is eligible for free or reduced meal programs by checking the appropriate box.
6. Provide your child's Racial/Ethnic Group and Primary Language by checking all applicable options.
7. Fill in your child's address and Middle School/Program Site information.
8. Select the program times your student will participate in: Morning, Afternoon, or Both.
9. Complete the Youth Medical Release/Historical section with any relevant medical conditions and emergency contact information.
10. Sign and date at the bottom of the form to finalize your submission.
