Submit your official immunization records You may also hand-deliver immunization records to the Office of Admissions and Records, located in the Fitzgerald Student Services building on the second floor. You may fax immunization records to (775) 784-4283. You may email immunization records to mynevada@unr.edu.
Official Transcripts: Have the Office of Student Records from all universities and colleges you attended send a copy of your official transcript to our office. Also acceptable are e-mail copies sent from the Office of Student Records to MARCNevada@unr.edu.
Drexel requires all entering full-time undergraduate and graduate students to complete an immunization record. Students can access this form by going to the Forms page. There is a $50 University processing fee, which must be paid by all students regardless of where the student receives vaccinations.
Ask your doctor, pharmacist or other vaccine provider for an immunization record form or download and use this form [4 pages]. Bring this record with you to health visits, and ask your vaccine provider to sign and date the form for each vaccine you receive.
Vaccination records (sometimes called immunization records) provide a history of all the vaccines you or your child received. This record may be required for certain jobs, travel abroad, or school registration.
