1. Click ‘Get Form’ to open the Model Release Form in our editor.
2. Begin by entering the Photographer's Name and Model's Name in the designated fields.
3. Upload the Model's Photo by clicking on the 'Place Model's Photo Here' area.
4. If the model is a minor, fill in the Parent, Guardian, or Legal Representative's Name.
5. Enter the Production Date, which is the date when the photographs were taken.
6. Review and check one of the boxes under MODEL/PARENT/GUARDIAN INFORMATION to confirm age or representation.
7. Sign in the Model’s or Parent's Signature field and provide their Permanent Address.
8. Complete the WITNESS INFORMATION section by filling in your name, signature, and date.
