Complete Block 12 by listing the Recognized Organization (RO) and/or Recognized Security Organization (RSO). If space is insufficient, note it at the bottom of the form.
Ensure all relevant certificates related to deficiencies are listed in Block 15a, including their issuing authority and dates.
If there are any deficiencies identified in Block 16, check 'Yes' and complete FORM B (CG-5437B) as instructed.
