How to use or fill out Masthead with our platform
- Click ‘Get Form’ to open the Masthead in the editor.
- Begin by completing the 'Employee Information' section. Fill in the employer name, group policy number, employee's full name, division/location, billing code, and social security number.
- Indicate the requested effective date for each coverage subject to Evidence of Insurability (EOI). Ensure all fields are filled accurately.
- In the 'Coverage(s) subject to EOI' section, select all applicable coverages and enter the current coverage amounts. If no coverage exists, indicate '$0'.
- Complete the 'Health and Personal History' section for all individuals applying for coverage. Provide accurate responses to ensure a smooth underwriting process.
- Finally, review your entries for accuracy and completeness before submitting. Sign and date where required.
