Click ‘Get Form’ to open the ced evaluation in the editor.
Begin by filling out the Personal Information section. Enter your last name, first name, middle initial, and any other names that appear on records. Provide your mailing address, phone number, email, gender, date of birth, and country of credentials.
For New Applicants, navigate to the Fees & Services section. Choose one type of evaluation: General Evaluation ($90), Course by Course Evaluation ($145), or CPA Evaluation ($190). Indicate the purpose for your evaluation by selecting from options like Freshman admission or Employment.
If you are a Returning Applicant, specify when CED last evaluated you and select any additional services needed. Review the fees associated with these services.
Complete the Educational History section by listing all institutions attended along with graduation years and degrees received.
Finally, review all information for accuracy and sign the application form before submitting it through our platform.
