1. Click ‘Get Form’ to open the Alberta Emergency Services Medal Nomination Form in the editor.
2. Begin by filling out Section A, 'Recipient'. Enter the recipient's surname, given names, gender, birth date, home address, city, postal code, rank, and MLA representative if known. Ensure all information is accurate.
3. In the same section, check all applicable disciplines related to the candidate's history such as Fire, Police, EMS, etc. Specify service dates by entering the month and year for both 'Service From' and 'Service To'.
4. Proceed to Section B to select the appropriate award type: AESM or one of the service bars (22-year, 32-year, or 40-year).
5. Complete Section C by providing your details as the nominator. Fill in your surname, first name/initials, position, municipality mailing address, telephone number, and email. Don't forget to sign and date this section.
6. If necessary, attach a letter of confirmation from each department listed in Section A before submitting.
7. Finally, forward the completed nomination form via email or mail to the appropriate addresses provided at the end of the form.
