4. Continue to fill out the Institutional/Dynamic Variables section, carefully assessing each statement regarding behavior and treatment history. Mark your selections clearly.
5. Once all sections are completed, calculate the subtotals for both Historical/Static and Institutional/Dynamic variables as instructed.
6. Finally, enter your name, date completed, phone number, and signature at the bottom of the form before saving or sharing it.
