5. Fill in the MOTHER’S FIRST, MIDDLE, & LAST NAME along with her date of birth and place of birth.

7. Indicate the child’s sex by checking either Male, Female, or Stillbirth as applicable.
8. Provide both parents' addresses and social security numbers in their respective sections for identification purposes.
9. Both parents must sign at the bottom of the form. If either parent is a minor, ensure a parent signature is included.
10. Finally, have a notary public witness your signatures and complete their section before submitting it to the State Office of Vital Records.
