4. Indicate where you are employed by filling out the 'Where employed' section.
5. In the 'Test administered by' section, print the name of the individual who conducted your test.
6. Select your ICS position for which the test is required and indicate the performance level needed by checking one of the options: Arduous, Moderate, Light, Field Test, or Walk Test.
7. Choose the type of test taken by selecting either Pack Test, Field Test, or Walk Test.
8. Once completed, have your test administrator fill in their details and certify that the test was administered according to agency guidelines.
