1. Click ‘Get Form’ to open the FSDD Enrollment Form in the editor.
2. Begin by entering your Employee Name and Social Security Number at the top of the form. This information is essential for identification purposes.
3. Read the authorization statement carefully. You must sign and date this section to authorize your employer to deposit funds into your accounts.
4. In the Account Information section, you can list up to three accounts. For each account, provide the Bank Name, Routing/Transit Number, and Account Number. Indicate whether it’s a Checking or Savings account.
5. Specify how much you wish to deposit into each account: either a specific dollar amount or the entire net amount owed to you.
6. Attach a voided check for each checking account listed. If using a savings account, ensure you have the correct Routing/Transit Number from your bank.
7. Once completed, submit the form to your payroll manager for processing.
