1. Click ‘Get Form’ to open the RTS-3 in the editor.
2. In Section 1, identify your tax account by entering your Account Name, RT Account Number, Mailing Address, Business Partner Number, City/State/ZIP, Tax Certificate Number, Email Address, Federal Identification Number, and Telephone Number.
3. Move to Section 2 to specify the Tax Type. Check the box next to 'Reemployment Tax' and any other applicable tax types you wish to change.
4. In Section 3, select the address type for your new address. Fill in the New Address Information including name of business or individual, Mailing Address, City/State/ZIP, Fax Number, Email Address, and Telephone Number.
5. Section 4 allows you to change your account status. Choose whether to Inactivate, Reactivate or Cancel your account and provide the effective date.
6. If applicable, complete Section 5 for a corporate name change by entering the new corporate name and effective date.
7. In Section 6, indicate if you are leasing employees. Provide necessary details such as Leasing Company’s RT Account Number or Federal Identification Number and start date of leasing.
8. Finally, sign and date in Section 7 certifying your authority to make these changes. Ensure all information is accurate before submitting.
