1. Click ‘Get Form’ to open the army extension document in the editor.
2. In SECTION I - SOLDIER'S REQUEST, fill in your details under 'FROM' and 'TO'. Select whether you wish to reenlist or extend your enlistment by checking the appropriate box.
3. For accrued leave options, choose one of the three options regarding cashing in your accrued leave. Ensure you understand each option before making a selection.
4. Enter the date and provide your signature at the bottom of SECTION I.
5. Move to SECTION II - COMMANDER'S CERTIFICATION. The commander will need to select their determination of qualification for your request. This section requires their input, so ensure they have access to complete it.
6. The commander must also indicate their recommendation for continued service within the Army by selecting one of the options provided.
7. Finally, ensure that the commander types their name, rank, and branch, followed by their signature and date at the end of SECTION II.
