2. Begin with Part A, selecting whether you want to assign a new MMSI, modify an existing one, or cancel an existing MMSI. Make your choice clearly.
3. In Part B, fill out the mandatory fields marked with 'U'. Start with your name (Last, First), telephone number (including country code), and mailing address.
4. Provide your email address and, if applicable, the nine-digit MMSI number beginning with 316. This is only required if previously issued.
5. Enter the vessel name and any previous vessel name if modifying. Complete the vessel owner information if different from your details.
6. Select the general classification and individual classifications from Annex E as required.
7. Fill in the gross tonnage of your vessel and provide emergency contact details including name, address, and telephone numbers.
8. Lastly, include radio installation information and vessel length before reviewing all entries for accuracy.
