What is included in the care plan?

Who is eligible for GPMP?

To be eligible for a GPMP, your patient must have a chronic or terminal medical condition. If they need ongoing treatment from a multidisciplinary team, theyre also eligible for TCAs.

Who is eligible for a Chronic Disease Management plan?

To be eligible, you must have: A chronic or terminal medical condition which has been present for 6+ months. A condition that requires ongoing treatment from a multidisciplinary team, with at least 2 other medical or allied health practitioners involved in your care (in addition to your GP)
