3. In the 'DIVE' section, enter the dive position number and select from options A, B, C, or D for each dive performed.
4. Provide a detailed description of each dive in the 'DIVE DESCRIPTION' field to ensure clarity for judges.
5. As scores are given by judges, input their scores in the 'JUDGES' SCORES' section for each corresponding dive.
6. Calculate and enter the total degree of difficulty (D.D.) for both voluntary and optional dives at the end of the form.
7. Finally, ensure all signatures are collected from the diver, coach, and referee along with their contact information before submission.
