1. Click ‘Get Form’ to open the US Person Validation Form in the editor.
2. Begin by entering your First Name, Middle Name (if applicable), and Last Name in the designated fields.
3. Input your Start Date to indicate when you will begin your employment.
4. Select one of the three acceptable documents for proof of US Person Status. You can choose from a Birth Certificate, US Passport, or Permanent Resident Alien Identification Card. Remember, photocopies are acceptable.
5. If you select one of the next three options (Certificate of Citizenship, Certificate of Naturalization, or if you do not have any of the referenced documents), ensure to provide your email address and phone numbers in the respective fields.
6. Once completed, fax this form along with your selected document to 513-243-6780 or scan and email it to itc.uspv@ge.com five days prior to your start date.
