1. Click ‘Get Form’ to open the youth player registration application in the editor.
2. Begin by filling out the Parent/Guardian Information section. Ensure you complete all required fields marked with an asterisk, including First Name, Last Name, and Email.
3. Next, provide Player Information. Indicate if the player is new or returning, and if returning, enter their Cal South Player ID Number. Fill in the player's First Name, Last Name, Date of Birth (DOB), School Name, and Grade.
4. Complete additional details such as League selection, Shirt Size, Play Type (Competitive or Recreational), Height, Weight, and Age Group. Make sure to check the Gender field for accuracy.
5. In the Emergency Contact section, list two contacts along with their phone numbers. If applicable, note any medical problems or physical limitations.
6. Finally, review the Cal South Waiver and Roster Freeze sections. Ensure you understand and agree to the terms before signing as a parent or legal guardian.
