- Once all sections are filled out, ensure that signatures from both the Skater and Contractor are obtained. If applicable, include a parent or guardian's signature for skaters under eighteen.
- Submit your completed ESCA to USFS Headquarters via email or fax as indicated in the document, ensuring you include any required processing fees.
