4. Complete the address section with your house number, street, area, town/village, and PIN code. Ensure all details are accurate for effective processing.
5. Select the reason for applying for a duplicate card by ticking the appropriate box. You can choose to collect it from designated centers or receive it by post.
6. Sign the application at the designated area and date it accordingly. Make sure to return any old or mutilated cards if applicable.
