Fill in the 'Exercise Time' fields to document how long you spent on each activity. Use checkboxes for light, moderate, or vigorous intensity as applicable.
Rate your health habits satisfaction for exercise and diet using a scale from high to low. This reflection helps assess your overall wellness.
At the end of each week, calculate and enter your total hours of physical activity and have a parent/guardian sign for verification.
