Calculate and enter the 'Total hours' of all courses presented for transfer at the bottom of this section.
Read through the policy statement regarding continuous enrollment, then sign and date the form to confirm your understanding and compliance.
If applicable, have the Department Head or Director of Graduate Study complete their section, including their signature and date.
