2. In arrowbox 1, enter the name of the county where you are filing, ensuring it matches what you wrote on Form 1.
3. In arrowbox 2, input your full name exactly as it appears on Form 1.
4. In arrowbox 3, write your spouse's name as recorded on Form 1.
5. Enter the docket number in arrowbox 4, which is provided by the Prothonotary's Office from Form 1.
6. In arrowbox 5, specify the name of the adult who served your spouse with Form 1 (this cannot be you).
7. After service is completed, have the third party write the date of service in arrowbox 6.
8. The third party should then enter today’s date in arrowbox 7.
9. Finally, have them sign in arrowbox 8 to complete the affidavit.
10. Once filled out, file this form with the Prothonotary and ensure a copy is provided to the other party.
