  6. In the Hardship Affidavit section, check all applicable reasons for financial difficulties and provide any necessary explanations.
  7. Complete the bankruptcy information if applicable, including filing date and case number.
  8. Fill out the government monitoring information as desired; this section is optional.
  9. Review your entries for accuracy before signing at the bottom of page two.
