How to use or fill out the ast form with our platform

- Input your 'AST Account Number', which is a 10-digit numerical code found on your account mailings.
- Fill in the 'Current Registration' as it appears on your account. This ensures that the transfer is correctly attributed.
- Select a 'Transfer Reason' by checking one of the provided boxes. If no reason is selected, transfers will default to gifts.
- Complete the 'Date' and provide signatures of assigners along with their capacities if applicable. Include contact information like daytime phone number and email address.
- For Medallion Signature Guarantee, ensure all signatures are guaranteed by an eligible institution, or obtain a waiver if applicable.
- Proceed to fill out the 'New Registration' section, including full name and total shares being transferred.
- Complete the Substitute Form W-9 by entering your Taxpayer Identification Number (TIN) and signing where indicated.
