Quality-of-Life Assessment. The KCCQ-12 is the short version (12-item) of the Kansas City Cardiomyopathy Questionnaire (KCCQ) (23-item). This self-administered test measures symptoms, physical and social limitations, and quality of life in patients with HF.
As described, all KCCQ scores are scaled from 0 to 100 and frequently summarized in 25-point ranges, where scores represent health status as follows: 0 to 24: very poor to poor; 25 to 49: poor to fair; 50 to 74: fair to good; and 75 to 100: good to excellent.
Recommended threshold scores have been reported for poor, fair, good, and excellent levels of functioning on the KCCQ overall summary score (OSS): 25, 25 to 50, 50 to 75, and 75 to 100, respectively.
KCCQ-12 is a short version of the original 23-item instrument and preserves the high test-retest reliability, responsiveness, prognostic ability, and interpretability thresholds of the original KCCQ (17).
The KCCQ-12 is a validated health status measure for patients with HF. 1012. It contains four subdomains: Physical Limitation, Symptom Frequency, Quality of Life, and Social Limitations. Each subdomain provides an individual score from 0 to 100, with 0 denoting the worst and 100 the best possible health status.

The responses are categorized under 3 subscales (symptom burden, physical limitation and quality of life) with a range of possible subscale scores from 0 to 100, with 100 representing the least burden of symptoms. The total KCCQ score represents the mean of the three subscale scores.
The KCCQ-12 is a shorter version of the original 23-item instrument that should be more feasible to implement while preserving the psychometric properties of the full instrument.
