1. Click ‘Get Form’ to open the K-12 TAP Card Application in the editor.
2. Begin with Section I, where you will enter your personal information. Fill in your last name, first name, middle name/initial, street address, apartment number (if applicable), city, state, zip code, telephone number, birth date, and the name and address of your school.
3. Proceed to Section II to confirm eligibility. Indicate your current grade level by selecting either 'Student 9-12' or 'Student K-8'. Provide the month of promotion to the next grade for verification.
4. Attach one of the required documents such as a photocopy of your current report card or school ID. Ensure all fields are completed accurately before submission.
5. Once finished, save your application and submit it either in person at a Metro Customer Center or mail it to the TAP Service Center.
