6. Complete Section E: RENT OBLIGATION AND ARREARAGE by detailing your current rent obligation and any arrears since March 1, 2020.
7. Fill out optional demographic information in Section F if comfortable, then review and sign Section G: APPLICANT CERTIFICATION AND AUTHORIZATION.
8. Once completed, submit the application along with verification documents as instructed within 30 days.
