There are a number of variations to the standard Snellen eye chart. The one an eye doctor uses depends on the personal needs and abilities of the patient. For example, eye doctors will use charts with pictures or patterns for younger children who may not have learned to read or identify letters and numbers.
If corrective wear (if needed) cannot allow you to reach a level of visual acuity better than 20/200 vision (where at 20 feet you see a level of detail a person with normal vision can see at 200 feet) in at least one eye, you cannot drive in California.
The third line is equivalent to 20/40, it is the driver's test line. You must be able to read most of the letters on this line in order to obtain an unrestricted drivers license in most states including Ohio.
There will be no lens restriction if you passed the vision test without glasses or corrective contact lenses. If you do not pass the vision test, DMV will schedule a Supplemental Driving Performance Evaluation (SDPE) (driving test) for you to see if you can compensate for your vision condition.
Having vision in just one eye is called monocular vision, and is actually perfectly legal for driving.

If you are wearing glasses and your driver license is not restricted, the employee will ask you to remove your glasses. The DMV does not want to restrict your driver license if it is not necessary. If you have difficulty reading the eye chart, the employee will take you to a vision testing machine.
