1. Click ‘Get Form’ to open the soccer team registration form in the editor.
2. Begin by entering your team's name in the designated 'TEAM NAME' field. This identifies your group for the tournament.
3. Select the age category for your team by checking the appropriate box (11, 12, 13, or 14) under 'AGE'.
4. Indicate the gender of your team by selecting either 'F' for female or 'M' for male.
5. Specify your affiliation by choosing between USYSA or other options in the 'AFFILIATION' section.
6. Fill out the 'TEAM CONTACT INFORMATION' section with the contact person's name, cell number, address, and email.
