Begin by filling out Section 1: Personal Details. Enter your Family Name, First Name, Middle Name(s), Date of Birth, Student ID, Telephone, and Email.
Proceed to Section 2: Request Details. Indicate the reason for your request by selecting from options such as 'New card', 'Damaged card', or 'Lost/Stolen ID card'. If applicable, provide payment numbers where required.
In Section 2: Mailing Address, enter your complete address including Number, Street name, Suburb, State, Post code, and Country.
Section 3 requires you to consent to verify your details via the Document Verification Service (DVS). Confirm your authorization and consent accordingly.
Finally, read and sign the Declaration in Section 4. Ensure all information is accurate before submitting.
Email the completed form along with certified documentation and a recent color passport-style photo to student.id@unsw.edu.au.

Using RFID proximity card technology, your university college ID cards can support physical access control to your campus buildings, such as dorms, or even specific areas in your campus buildings, such as labs.
How do I get my UB ID card?
Go to a UB Card Office during their hours of operation to have your picture taken. You must have photo ID - either a valid drivers license or a passport - in order to receive your UB Card. Photos may also be submitted online at Get My UB Card page.
Where do I get my UB ID card?
In-person. The UB Card Office issues all cards. The North Campus location is 1 Capen and the South Campus location is 1 Diefendorf Hall. There is a nominal fee.
How can I get UIC ID card?
We encourage patrons needing i-cards to first attempt to submit a photo online using SubmitMyPhoto. Allow a few days for photo review. If you cannot submit a photo, contact idcenter@uic.edu for more information or visit our office to have your card issued in person.
