The two-step tuberculin skin test (TST) is used to detect individuals with past tuberculosis (TB) infection who now have diminished skin test reactivity. This procedure will reduce the likelihood that a boosted reaction is later interpreted as a new infection.
The skin test reaction should be read between 48 and 72 hours after administration....The reaction should be measured in millimeters of induration (palpable, raised, hardened area or swelling). Do not measure erythema (redness). The indurated area should be measured across the forearm (perpendicular to the long axis).
A 2 step is defined as two TST's done within 3 months of each other. The optimal time for testing would be to complete the 2 TST's within 1-4 weeks of each other. In some persons who have had an unknown, past exposure with M. tuberculosis, their body's ability to react to a TST may wane over time.
"Reading" the skin test means detecting a raised, thickened local area of skin reaction, referred to as induration. Induration is the key item to detect, not redness or bruising. Read skin tests 48-72 hours after the injection when the size of the induration is maximal.
