For section (B), specify the state where your corporation was formed.
Fill in section (C) with the complete address of your corporation.
In section (D), provide the name and address of your resident agent in Maryland, as this is a requirement for foreign corporations.
If applicable, complete section (E) with the address of your principal office in Maryland. This step is optional.
In section (F), check the appropriate box to indicate whether your corporation has conducted business in Maryland prior to qualification. Remember, if you check 'Has', include an additional penalty fee.
Finally, ensure that both the President or Vice President and the Resident Agent sign where indicated at the bottom of the form.
