You can get the Certification of Disability for Disabled Parking Placard and/or Plate (Form TR-159) by going to your County Treasurers Local Motor Vehicle Office or printing the document online.
To apply for a disability plate or placard, you must: Complete an Application for Disabled Parking. Your healthcare provider will have to fill out a section to verify your disability status.
