5. Move on to Sections Four through Six where you will assess various competencies of the applicant. Provide detailed comments based on your observations regarding their medical knowledge, organizational skills, communication abilities, professional attitudes, fitness to practice, strengths and weaknesses.
6. If applicable, complete Section Seven for applicants applying at a specialist level by commenting on their training and competence.
